USD 409 MEDICATION ADMINISTRATION FORM

USD 409 IMMUNIZATION REQUIREMENTS

USD 409 follows the immunization requirements set forth by the Kansas Department of Health and Environment.

 

THE FOLLOWING IMMUNIZATIONS ARE REQUIRED for 5 year olds,  Kindergarten, 1st,  2nd and 3rd  graders for 2007-8 school year:  (Requirements progress a grade each year. For example, K, 1st, 2nd, 3rd, and 4th will follow requirements for 2008-09)

 

Entering preschool and kindergarten students must have a copy of his/her immunization record on file at school before attendance will be allowed.  This record should be turned into the school nurse at summer enrollment or to the school office.

 

DPT/DTaP: At least 4 doses:  a fifth dose is required if the fourth DPT/DTaP was given before the child’s fourth birthday.

 

IPV/OPV:  At least 3 doses; a fourth dose is required if  (a) the third IPV/OPV was given before the child’s fourth birthday, or,  (b) if a combination schedule of IPV/OPV.

 

MMR:  2 doses are required; the first dose must have been given after the child’s first birthday.

 

HEPATITIS B (HBV):  3 doses are required; there should be 4 wks between dose 1 and 2; 8 weeks between dose 2 and 3 and at least 4 mo between dose 1 and 3.

 

VARICELLA  (Chicken Pox):  one dose after 1st birthday required unless proof of immunity is verified by signature of physician or parent indicating history of disease and date of illness or lab evidence of immunity; varicella can be given at the same time as the MMR or one month before or after the MMR. NOTE: TWO doses are now required for Kdg and 1st graders beginning 2007-8 school year: second dose to be given at 4-6 years of age. The second shot should be given at least 3 mo. after the first shot if under age 13. This varicella requirement will progress each year.

GRADES 4 AND ABOVE:  Immunizations listed above, except HBV and varicella, are required for grades 4 and above for 2007-08 school year.  Requirements for HBV and variciella progress each year.  For example, HBV and varicella will be required for Kdg, 1st, 2nd, and 3rd  and 4th  for 2008-09 school year.

 

Td (Tetanus/diphtheria) or Tdap:  The Td booster is required 10 years after completion of the DTP/DTaP/DT/Td primary series.  For example, if a student received his/her last Dtap at age 5, a  Td or Tdap is required at age 15.  This record should be given to the school nurse at summer enrollment or to the school office.

 

HEALTH ASSESSMENTS:  (forms are available at schools)

 

AGE 8 OR UNDER:  Kansas Law requires that students eight years old and under have a health assessment (physical) on file at school.  This health assessment may be done by your child’s doctor or at a Health Department.  The school will also accept a copy of a health assessment done for entry to a Kansas Pre-school or a recent KanBeHealthy Assessment for children with Medicaid.  If this is the first entry into a Kansas School, the physical should be one done within the past 12 months.

 

SIXTH GRADE STUDENTS:  USD 409 policy requires all sixth grade students have a medical examination performed by a licensed medical physician (MD) or Physician’s Assistant (PA).  A copy of this health assessment should be turned into nurse at summer enrollment

 

SPORTS PHYSICALS:  The Kansas State High School Activities Association requires an annual physical done by an MD, or Physician’s Assistant.  Physicals can be completed any time after May 1 for the upcoming school year.   A copy must be submitted to the Athletic Director for each student each year of participation.
